Release and Waiver of Liability

I, the undersigned, being the parent or legal guardian of the below-named child, do hereby acknowledge and agree to the following:

1. Assumption of Risk: I acknowledge that participation in the programs, activities, and events organized by the KRoussaw Foundation (hereinafter referred to as "the Foundation") involves inherent risks, including the risk of physical injury. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by my child, or any loss or damage to property owned by my child, as a result of participation in such activities, whether caused by the negligence of the Foundation or otherwise.

2. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Foundation, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, or to any property belonging to my child, while participating in activities, or while on the premises where the activities are being conducted.

3. Indemnification: I agree to indemnify and hold harmless the Foundation from any loss, liability, damage, or costs, including court costs and attorneys’ fees, that they may incur due to my child’s participation in said activities, whether caused by negligence of the Foundation or otherwise.

4. Medical Treatment Authorization: In the event of an emergency, I hereby authorize the Foundation to seek and obtain medical treatment for my child. I understand that I am responsible for any medical expenses incurred.

5. Behavioral Expectations: I acknowledge and agree that my child must abide by all rules and regulations set forth by the Foundation. I understand that the Foundation reserves the right to dismiss any participant who does not adhere to the rules or exhibits behavior deemed inappropriate or unsafe.

6. Photographic Release: I understand that the Foundation may take photographs or video recordings of the participants during the activities for promotional and educational purposes. I consent to the use of my child's image in such materials.

By signing below, I acknowledge that I have read and understand this General Liability Release Form. I fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
