KRF Photo Release Form

I, the parent or legal guardian of the below-named child, hereby grant permission to the KRoussaw Foundation, its representatives, and employees the right to take photographs, video recordings, and/or digital images of my child in connection with the activities and programs of the KRoussaw Foundation.

I authorize the KRoussaw Foundation to use, reproduce, and/or publish these photographs and/or videos, including their digital versions, in perpetuity, for any lawful purpose, including but not limited to publicity, fundraising, advertising, social media, and educational purposes, without compensation or any other consideration to me or my child.

I understand and agree that these materials will become the property of the KRoussaw Foundation and will not be returned.

I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child’s likeness appears. I waive any right to royalties or other compensation arising or related to the use of the photographs or videos.

I hereby hold harmless and release and forever discharge the KRoussaw Foundation from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

By signing below, I acknowledge that I have read and understand the terms of this photo release consent form.
